JOB SUMMARY
The Part-time Staff Physician provides direct patient care focusing on primary care, prevention, education, and coordination of behavioral health, social services, lab work and imaging, and referral to specialty care when deemed clinical necessary. The staff physician works cooperatively and effectively with the other physicians, providers, dentists, clinical support staff (including nurses and medical assistants) to ensure overall health care delivery meets Patient Centered Medical Home standards, Joint Commission Standards, HRSA standards, Meaningful Use and other measurements of quality care, patient safety and patient satisfaction.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Clinical Functions
· Provide routine primary care, urgent ambulatory, patient education and coordination of behavioral health services.
· Practice medicine within license and training; diagnoses and treats diseases and disorders of the human body.
· Examine patients utilizing all types of instruments, medical equipment and tests following standard medical procedures/protocols.
· Complete all meaningful use measures or any other clinical quality metrics (including population health metrics) as defined by CMS, participating insurance health plans and other governing bodies.
· Participates in activities to support Patient Centered Medical Home (PCMH) status, Quality Assurance / Quality Improvement activities, Corporate Compliance, Risk Management, OSHA standards.
· Complete all NCQA and Joint Commission patient care standards.
· Prescribe and administers medications as needed.
· Provide after hour coverage per center policy and schedule.
· Utilize electronic health record as the primary source of documentation and enter information in the correct areas within the health record software.
· Provide all necessary and appropriate coding related to the care of the patient.
· Attend business related meetings, public/private partnership opportunities on behalf of Foremost as assigned.
· Establishing and strengthening referral relationships with specialist providers, hospitals, and other entities
SUMMARY:
A nurse practitioner is an advanced practice nurse that helps with all aspects of patient care, including diagnosis, treatments and consultations. They may work in both inpatient and outpatient situations and can perform independently or as part of a treatment team. Generally, nurse practitioners perform the important task of educating patients about preventative care and the prescribed treatments. They may also conduct physicals, order tests and serve as a patient's primary healthcare provider. Some nurse practitioners are also able to prescribe medications. While nurse practitioners perform independently in the community health center, it is the expectation that the nurse practitioner assesses, diagnoses, and provides appropriate treatment to patients, and when necessary, consult with the physician(s) at the health center who have the delegated prescriptive authority for that nurse practitioner. The nurse practitioner operates under that physician’s prescriptive license and must provide the minimum expected charts to be reviewed by the delegated physician on a regular basis. The Nurse Practitioner – School-Based (YES Prep) Health Program will be responsible for administering care to children within the School-Based (YES Prep) program clinic setting, as well as family members of enrolled students in the YES Prep Program under the supervision of physicians.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Patient Visits and Patient Panel
· Minimum encounters: average 20 per day or minimum 400 per month
· Minimum patient panel: 750 – 1,200 patients to manage
Clinical

· Confer with medical and dental providers on patient conditions
· Coordination of Hospitalization if necessary

· Function as the patient’s primary care provider at the health center.

· Perform RN skills and duties when necessary
· Plan of Care: Primary Care according to specialty; Complete medical history; Medical examination and diagnosis; Treatment and/or referral.
· Prescribe appropriate medications
· Preventive health education
· Referrals to specialist if necessary, and follow up after the specialty visit to ensure the patient
· Supervise medical assistants, LVNs and RNs if needed
· Triage patients for urgent or emergency care
· Vital Signs (height, weight, blood pressure)
Clinical Quality, Infection Control, Case Management, JC/PCMH
· Direct policy writing and implementation of infection control activities and compliance
· Review care coordination activities and document activities as case management.
· Provide clinical case management.
· Direct policy writing and implementation of clinical quality measures, data gathering and performance.
· Coordinate and implement activities to secure and maintain PCMH / Joint Commission accreditation and recognition.
Administrative
· Chart Organization, Retrieval, Filing
· Clinical protocols development, writing, implementation, compliance
· Coding with appropriate diagnosis and level of service coding (ICD9 or ICD10 and CPT)
· Collaboration with other department staff
· Complete encounter forms on each patient
· Department budget management

Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories).
