POSITION OVERVIEW
The Nurse Practitioner engages in the professional, organizational, and administrative activities to provide quality care to clients based upon their medical and psycho-social needs. The Nurse Practitioner along with the medical director, can develop, implement, and evaluate the medical practice delivery model in accordance with specific goals. The responsibilities of the Nurse Practitioner are leadership, direction and coordination in the provision of medical care and physician services supervised by the medical director and program director. The Nurse Practitioner assumes responsibility for applicable Federal, State and City regulations for the department.
KEY ESSENTIAL FUNCTIONS
POSITION OVERVIEW:
The Registered Nurse (RN) is accountable for evaluating consumer’s needs for nursing care on an ongoing basis. The RN is responsible for the physical health of Intensive Mobile Treatment (IMT) consumers. The functions below outline the responsibilities associated with this position. These functions are considered critical to effective department operations and Acacia’s mission. Satisfactory attendance and performance of functions in a competent, cooperative, and timely manner is required.
KEY ESSENTIAL:
• Conduct initial health screenings for IMT consumers.
• Provide referrals as needed to primary and/or specialty medical care.
• Conduct blood draws and administer psychotropic medication treatment, including administration of IV medications.
• Prepare medication to be dispensed, and work closely with the Psychiatric Nurse Practitioner to dispense medication and monitor clients’ physical and mental wellbeing.
• Conduct care coordination between other IMT staff members, pharmacy, health homes, and other outside providers • Record patient data in Promesa’s NextGen Electronic Health Record system.
• Participate in grant related workshops and trainings.
• Must be able to deliver in-person services after standard hours of operation as needed.
• Perform clinical, compliance, and administrative duties as instructed.
• Enhance patient’s health and self- management skills as developing wellness goals in the use of evidence-based practices and maintain record of these activities.
• Participate in daily IMT staff and organizational meetings.
• Practice according to the principles of recovery oriented and trauma informed service-delivery.

Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories). Half-time service is also eligible at a reduced award. Eligibility extends to NPs, PAs, CNMs, CRNAs, and physicians with qualifying education debt.
