The Street Medicine Advanced Practice Provider (APP) delivers compassionate, patient-centered medical care to individuals experiencing homelessness and other vulnerable populations in community-based settings. This role requires flexibility, strong clinical judgment, cultural humility, and a passion for serving underserved communities through outreach-based healthcare delivery. The APP works collaboratively with an interdisciplinary team to provide primary care, chronic disease management, urgent care services, behavioral health support, care coordination, and health education directly in the field.

Requirements
Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment. Half-time service is also eligible at a reduced award (up to $37,500). Eligibility extends to NPs, PAs, CNMs, and physicians with qualifying education debt.
Alabama is a restricted-practice state — Nurse Practitioners require physician supervision or delegation for prescribing and diagnostic decisions.
