JOB RESPONSIBILITIES
• Conduct initial patient screenings and patient history and physical examinations.

JOB REQUIREMENTS
• Minimum of 2 years of clinical experience in outpatient care preferred
• Demonstrated ability to provide high-quality patient care
• Strong diagnostic and clinical problem-solving skills
• Proficiency in electronic medical record (EMR) systems (specifically EPIC)
• Excellent communication and interpersonal skills with patients, families, and healthcare team
members
• Ability to work independently with minimal supervision
• Commitment to continuing education and staying current with medical best practices and
standards
• Ability to work effectively in a multidisciplinary team
• Willingness to participate in on call as part of a rotation, if required
• Experience with telemedicine and remote patient care is a plus

INTRODUCTION
The Advanced Practice Provider (APP)—either a Physician Assistant (PA) or Nurse Practitioner (NP)—provides high‑quality, patient‑centered primary care services within the Federally Qualified Health Center (FQHC) at MidMichigan Health Services. Serving as a physician extender, the APP delivers comprehensive assessment, diagnosis, treatment, and follow‑up care for patients across the lifespan, with a strong emphasis on prevention, chronic disease management, and health equity. The APP practices in collaboration with supervising or collaborating physicians in accordance with federal, state, and organizational regulations. The APP functions as a member of the primary care team, working collaboratively with physicians and interdisciplinary staff to improve patient access, quality outcomes, and continuity of care. The APP provides direct patient care while supporting physician panel management through chronic disease management, preventive services, transitional care, and same-day access.

This position works as an LPC/LMSW therapist to provides a full range of effective Behavioral Health services to both adult and child patients. Participates in interdepartmental, intersystem and external committees, meetings and activities to promote the mission, values and business of MidMichigan Community Health Services.
JOB RESPONSIBILITIES
• Provides counseling services to individuals based on individual treatment plans. Plans treatment based on the individual’s needs and resources as well as on clinic and community resources.
• Maintains good communication and collaborative relationships with referring medical providers and other staff and recommends additional referrals for patients as needed. Coordinates with professionals involved in patient care to insure collaboration and continuity of services.
• Maintains comprehensive and current knowledge of community resources and assists patients as needed in accessing these resources as appropriate. Is available for consultation with other clinic staff regarding community resources.
