Nurse Practitioner - Geriatrics

Center for Pan Asian Community Services, Inc. is looking for a Registered Dental Hygienist to provide preventive care, oral health education, and therapeutic services to patients of all ages in accordance with state licensure requirements, organizational policies and FQHC standards. The hygienist works collaboratively with dentist, dental assistants, and other healthcare professionals to support integrated, patient-centered care for underserved and diverse populations.

Essential Duties and Responsibilities

• Perform oral prophy
• Review patients' medical and dental history
• Examine the teeth and surrounding tissues for evidence of plaque and periodontal disease and chart all fillings
• Inspect the mouth and throat for evidence of disease
• Perform routine x-rays and refer abnormalities to the dentist
• Prepare dental hygiene treatment plans and the sequence of appointments needed to complete treatment
• Polish teeth and apply stannous fluoride
• Educate patients on the need for daily oral health and fluoride
• Expose, develop, and process dental radiographs
• Conduct patient assessments and document findings in eCW
• Collaborate with dentists to implement individualized treatment plans
• Maintain compliance with OSHA, CDC, HIPAA, infection control, and FQHC regulatory standards
• Participate in community outreach programs as needed
• Support quality improvement initiatives and integrated care coordination efforts
• Maintain operatory equipment and dental supplies
• Participate in staff meetings, trainings, and continuing education activities
• As directed by the Chief Executive Officer, perform other related and/or necessary tasks to achieve organizational and programmatic goals and objectives.

Cosmo Health Center is seeking a Family Nurse Practitioner to expand our dedicated team of professionals! We are a Federally Qualified Health Center focused on working with the underserved that is uniquely poised to serve communities with limited-English proficiency. We are affiliated with the Center for Pan-Asian Community Services (CPACS), a non-profit organization that addresses the needs of refugees, immigrants and the underserved through comprehensive health and social services, capacity building, and advocacy. This is a great opportunity for a candidate who has an interest in working with low-income populations and addressing global health on a local level. This is an environment where collaboration and open discussion with the medical team is a priority; must be comfortable with presenting cases to supervising physician and seeking assistance for issues that seem out of his/her scope. Will be assisting to provide the following: preventive and acute care for adults and children; women’s health including basic pelvic exams and clinical breast exams/referral for screening mammograms; working with translators/patient navigators; strengthening referrals to our dental site; and evaluating for substance abuse risk and referring to our trained counselors for prevention and treatment as needed.

Current providers include internal medicine, pediatric, general surgery/general practice physicians; dentist/dental hygienist; and certified counselors. Outpatient only; on-call is telephone only. Typical schedule is 15-20 patients per day. Must have BLS; PALS/ACLS a plus; will need GA NP license prior to start date.

Background or interest in public health is a plus. Fluent Spanish and conversational level of other languages a plus.

Must speak Spanish!

• Provide management on all aspects of Health Programs, including planning, implementing and evaluating activities associated with the activities.
• Analyzing clinical performance data and implementing quality improvement projects,
• Participating in grant writing for Federal/State/Local grant applications,
• Ensuring that Center’s grant program deliverables and reporting deadlines are met within the timeframe
• Experience in public health or social work program planning, implementation, and evaluation
• Ability to work effectively in a team.
• Strong leadership skills and proficiency in public speaking, group facilitation, training, strategic planning and implementation, data compilation, and program development.
• Cultural competency and success working with diverse communities.
• Proficiency in Spanish, Mandarin or Cantonese, Vietnamese, Korean, Nepali, Burmese, or Kinyarwanda is a plus.
