Sunrise Community Health is seeking an Advanced Practice Provider (APP) to deliver high-quality medical services to patients in Weld, Larimer, and surrounding counties in northern Colorado. This role involves promoting and maintaining patient health under physician supervision, with a focus on direct patient care in an outpatient setting. The APP will rotate between clinics for coverage, contributing to the organization's mission of providing affordable healthcare.

Requirements
Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment. Half-time service is also eligible at a reduced award (up to $37,500). Eligibility extends to NPs, PAs, CNMs, and physicians with qualifying education debt.
Colorado is a full practice authority state — Nurse Practitioners can evaluate, diagnose, prescribe, and manage treatment without a collaborative or supervisory agreement.
