Southwest Care Center is currently seeking a full-time Behavioral Health Counselor for our Alameda Clinic located in Santa Fe, NM.
Position Details:
- Screen and assess patients for common mental and substance use disorders.
- Support and closely coordinate mental health care with the patient’s primary care provider and other SCC staff as appropriate.
- Provide patient education about common mental and substance use disorders and available treatment options.
- Monitor clients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
- Support psychotropic medication management prescribed by Psychiatric Practitioner, focusing on treatment adherence, attention to side effects, and effectiveness of treatment.
- Provide or facilitate in-clinic or outside referrals as needed.
- Document patient progress and treatment recommendations in EMR.
- Conduct regularly scheduled caseload consultation with the multidisciplinary team and communicate the resulting treatment recommendations to the patient’s PCP. These consultations will primarily focus on patients who are new to treatment or who are not improving as expected with recommendations for changes to treatment if needed.
- Facilitate patient engagement and follow-up with behavioral health care.
- Track patient follow-up and clinical outcomes. Document encounters in EMR and use the system to identify and reengage patients who may be lost to follow-up.
- Facilitate referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance use treatment).
- Complete relapse prevention plans with clients with substance use disorders.
- Participate in regular training and technical assistance activities for SCC staff.
- Provide chart review or other clinical supervision for BHC I staff as assigned.

Southwest Care Center is currently seeking a full-time RN Care Coordinator working in our Alameda Clinic located in Santa Fe, NM. This is a Remote Hybrid Position. This position operates on a three day in clinic, and two days remote schedule.
Position Details:
- Serves as a key member of the interdisciplinary care team to identify high risk, high acuity patients that would benefit from care coordination.
- In collaboration with other providers and team members, develops interdisciplinary care plans for identified patients.
- Orients and educates patients (and family/support members as identified) by explaining the care coordination process and initiating their care plan.
- Coordinates care information and requirements with other care providers, resolves issues that could affect progression, and fosters peer support and education to others regarding the care coordination process.
- Continually evaluates adherence to and effectiveness of the care plan toward desired outcomes, modifying as necessary, to ensure the best outcome for the patient. Measures intervention effectiveness.
- Interacts both inside of SCC as well as externally to facilitate appointments, scheduling and completion of tests or other procedures, and to remove or minimize other barriers to adherence to care.
- May provide clinical care in compliance with SCC policy as well as the provisions of the New Mexico Nurse Practice Act.
- In collaboration with treating provider, refers patients to additional providers or resources to support specialized needs.
- Documents appropriately and timely in the EMR.
- Collects and analyzes data to support development and improvement of clinical best practice and SCC Quality programs.

Southwest Care Center is currently seeking full-time Primary Care Medical Assistant's to work at our Clinics in Santa Fe & Albuquerque, NM.
Position Details:
- Screens patients according to established protocols (i.e., obtaining vital signs, height, weight, vision/hearing testing) and any other procedures directed by the supervising provider/Clinic Manager.
- Performs accurate charting in medical record in accordance with established SCC policies and procedures.
- Provide assistance to nurses and providers, as needed.
- Maintain exam rooms and work areas (i.e., keeping well‐stocked, orderly, and clean, to include disinfecting exam tables between patients as per infection control policy).
- Perform phlebotomy/fingerstick, as permitted by training.
- Administers injections according to position requirements and as trained.
- Perform procedures, as ordered, and trained.
- Able to accurately make/cancel/reschedule appointments in the appointment system.
- Able to follow‐up on appointment to determine patient compliance and contact patients to reschedule (no‐show policy).
