Position Summary: Under limited supervision by the Chief Medical Officer, the Psychiatric Nurse Practitioner or Physician Assistant provides comprehensive psychiatric care, including assessment, diagnosis, and treatment of patients with mental health disorders. Works collaboratively with physicians, therapists, and other health professionals to deliver integrated behavioral healthcare. Provider functions in accordance with his/her collaborative practice agreement signed by the collaborating physician as required by the South Carolina Nurse Practice Act/South Carolina Board of Medical Examiners. Responsibilities include
- Conduct psychiatric evaluations and mental status examinations.
- Diagnose and manage mental health conditions such as depression, anxiety, bipolar disorder, and schizophrenia.
- In collaboration with the primary care provider, develop individualized treatment plans, including prescribing and monitoring psychotropic medications.
- Provide crisis intervention and ongoing psychotherapy as appropriate.
- Complete timely documentation in the EMR for each patient encounter and review patient medical records for accuracy and completeness, ensuring timely medical record documentation.
- Requests, obtains, and evaluates results of screening tests.
- Monitors the treatment regimen in selected populations.
- Demonstrates proficiency in all the above. Assessed by medical and clinical preceptors through record audits and one-on-one training and observation, at least once a year, or if needed.
- Confer with collaborating physician when needed for consultation, information, etc.
- Conduct quality assurance review utilizing clinical measures by life cycle.
- Perform duties and support practice transformation as a Patient Centered Medical Home
- Serve as an advocate for the center and serve as liaison to local hospitals and health organizations as appropriate.
LocationThis position will work at several CareSouth Carolina locations. Location assignment will be prioritized by the volume of patients requiring the care. Two days per week will be in Hartsville, SC

- Primary responsibility is to work as part of CareSouth’s provider team performing a variety of direct patient services including assessment, diagnosis, treatment, and evaluation/monitoring as needed. The incumbent performs these functions in a primary healthcare setting. The physician may also perform specialty services after training, credentialing, and certification.
- Provide primary care to patients, including histories, exams, lab tests, prescriptions, and referrals within prescribed up-to-date guidelines and protocols.
- Perform accurate and timely documentation of histories, subjective findings, physical exams, plans, prescriptions, and follow-up in the Electronic Health Records System within 48 hours of the visit.
- Ensure that health education is provided with each patient encounter when applicable.
- Provide patients with information concerning their evaluation, diagnosis, treatment, and prognosis. Engage patients in participation and decision-making involving their healthcare. Provide counseling to patients on diet, hygiene, and preventive healthcare.
- Collaboration with clinical and other staff to achieve improved patient care & outcomes
- Provide timely coordination of care among internal and external ancillary providers and hospitals to provide high-quality care between departments and facilities
- Prioritize Value-Based Care at all patient visits, including (but not limited to):
- Hypertension control (BP <140/90)
- Diabetes under control (HbA1c <9)
- Breast cancer screening
- Cervical cancer screening
- Aspirin prescribed for patients with coronary artery disease
- Lipid management in patients with coronary artery disease
- Attend CareSouth meetings: site meetings, department meetings, and provider meetings.
