Nurse Practitioner - Family Practice

The Physician is a member of the interdisciplinary health team of FHCSO and works in collaboration with the Medical Director/Clinical Director, and their physician team, as well as supervising advance practice providers who provide primary care. The Physician provides advance practice providers with consultation as needed, provides primary care for clients in collaboration with other center physicians. The Physician also teaches health professions, students and residents.

We are looking for a Dentist to join our amazing team here at the Family Health Center of Southern Oklahoma. FHCSO is a Community Health Center that focuses on wellness, collaboration, and excellence. We provide care for all patients whether they have insurance or not and offer comprehensive services like medical, dental, prescription and mental health services, striving to treat the whole body.

We are open to candidates interested in full time, part time, and/or PRN work. Our current hours of operation are Monday-Thursday, 7:30am-6pm and Friday, 7:30am-11:30am.

Provides treatment for patients with psychiatric needs along the lifespan in an outpatient setting. Treatment includes assessing, planning, implementing, coordinating, and evaluating the patient care among a multi-disciplinary team.

A Licensed Clinical Social Worker (LCSW) is a member of the interdisciplinary health team of FHCSO and works in collaboration with the Medical Director/Clinical Director, Behavioral Health Director and their primary care teams. The LCSW provides quality counseling for clients in collaboration with other center physicians/counselors.

JOB SUMMARY:

The Nurse Practitioner/Physician Assistant is an integral member of the provider team, provides direct medical services to patients under direction and responsibility of a physician.

RESPONSIBILITIES:

1. Provides direct patient care, performs comprehensive physical examination, obtains a thorough and appropriate health history, compiles patient medical data and arranges testing, therapy, and follow-up.
2. Administers or orders diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests, and interprets test results for deviations from normal.
3. Performs therapeutic procedures such as injections, immunizations, suturing and wound care, and managing infection.
4. Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care.
5. Instructs and counsels patients regarding compliance with prescribed therapeutic regimes, normal growth and development, family planning, emotional problems of daily living, and health maintenance.
6. Documents each patient encounter in the patient record, clearly and concisely, on the day the patient encounter takes place.
7. Serves as a participant on rotational call schedule with other clinicians.
8. Participates in provider team meetings to comply with PCMH team standards.
9. Participates in any other designated activities and projects for PCMH as assigned by the Chief Medical Officer.
10. Initiates patient referrals as necessary, and communicates with the referral clinician or agency.
11. Supports activities related to the Health Disparity Collaborative. Duties include but are not limited to supporting the incorporation of staff time for data entry, team meetings, production and submission of monthly reports, maintenance of database.
12. Maintains and improves clinical skills by attending health center provider meetings.
13. Consults supervising or alternate physician when appropriate.
14. Participates in community agencies and/or resources in providing continuity of care.
15. Maintains up to date required state licensures and certifications.
16. Follows the mission and vision statement.
17. All other duties as assigned.
