POSITION SUMMARY:
The Crisis Clinician is responsible for providing comprehensive, client-focused mental health assessment and treatment utilizing individual and group modalities to people living with HIV/AIDS; LGBTQ+ clients; and the community at large. The Crisis Clinician is part of the Urgent & Integrated Care Team that operates within Equitas Health medical clinics. Crisis Clinicians collaborate with medical providers and other staff to provide care to clients who are presenting with urgent mental health concerns. The Clinician is able to assess the client’s symptoms, create a safety plan, determine a diagnosis, recommend a level of care, and refer the client for additional or ongoing mental health treatment, such as inpatient hospitalization, PHP/IOP, or outpatient counseling. The Clinician will be responsible for scheduling and conducting short-term solution-focused counseling focusing on safety and other immediate needs. The Clinician can also assist with other functions and roles needed in the Mental Health and Recovery Department, such as triaging referrals, providing gender affirming surgery letters, and bridging care while Clients wait to be placed with a long-term therapist. The Crisis Clinician will operate in accordance with established professional standards and guidelines as stated by the Ohio Revised Code and put forth by the Ohio Counselor, Social Work and Marriage and Family Therapist Board. The individual will operate in accordance with the established professional standards and guidelines for the National Association of Social Workers (NASW) or American Counseling Association (ACA) and agrees to adhere to NASW or ACA standards.

ESSENTIAL JOB FUNCTIONS:
Essential functions of the job include but are not limited to: diagnosis and treatment of clients with mental health and substance use concerns, crisis intervention and consultation, documentation in an electronic medical record system, utilizing telehealth, phone, and in person visits with clients, attending meetings, and producing clear, understandable, and timely documentation.

POSITION SUMMARY:
The Health Information Management Specialist (HIMS) is responsible for ensuring that the Equitas Health meets the required standards to have a successful records operation. Job Responsibilities:
- Direct the maintenance of all patients’ medical records in accordance with internal policies and with state and federal regulations.

Education/Licensure
- An Associate's degree from an approved program for Health Information Management, Business or related field OR two years of Medical Records experience preferred.
- College coursework or vocational/technical training in nursing or another health-related field may substitute on a year-for-year basis for the required experience.
- Bachelor's degree is preferred.
- Current credentials as a Registered Health Information Technician (RHIT) and/or Registered Health Information Administrator (RHIA) or equivalent certification around medical records management preferred.

POSITION SUMMARY:
The Prior Authorization Specialist handles all prior authorizations, third party appeals, and pending third party payer issues across all Equitas Health medical centers. This individual will work with internal and external pharmacies, CoverMyMeds, the EHR, and all Equitas Health health center providers to facilitate prior authorization completion. This individual must be able to work in conjunction with the pharmacy department and health center staff to facilitate the prior authorization process.
ESSENTIAL JOB FUNCTIONS:
The Prior Authorization Specialist is responsible for handling all prior authorizations, appeals, and third party medication disputes for providers at Equitas Health. The Prior Authorization Specialist will be required to communicate with prescribers, pend medication changes, and maintain exceptional documentation in the patient health record. The staff member will also be in constant communication with internal pharmacies, external pharmacies, and patients waiting on prior authorizations. Essential job functions include, but are not limited to, driving or having reliable transportation, written and verbal communication, utilizing a computer for typing, multi-system use/maintenance, and attending meetings.
MAJOR AREAS OF RESPONSIBILITIES:
- Complete prior authorizations submission within established time frame.
- Collaborate with providers to manage appeals and/or peer-to-peer discussion with third party representatives.
- Ensure proper documentation is maintained in the EHR.
- Work with clinical pharmacist team to provide potential therapeutic substitutions to prescribers
- Communicate with the clinical team on the status of prior authorizations or changes that must be made for therapy to continue for the patient.
- Maintain constant communication with the internal and external pharmacy teams for medication processing.

- Provide data on prior authorizations as needed to Senior Leadership.

The Care Right There (CRT) Therapist is responsible for providing comprehensive, client-centered mental health treatment to clients who are living with HIV/AIDS. Care Right There is a Ryan White Part A funded mental health treatment program focusing on meeting the needs of clients who have identified mental health needs but are unable to access clinic- or office-based treatment due to symptoms and other psycho-social stressors. The CRT Therapist provides treatment to clients in the home, in the community, and via telehealth in an effort to reduce symptoms and barriers that impact the client’s ability to participate in office-based outpatient therapy. The CRT Therapist will complete diagnostic assessment with treatment recommendations and will assist with the client in developing measurable treatment goals. POSITION SUMMARY
Reporting to the Clinic Manager, the Patient Access Representative (PAR) Lead serves as the operational lead for Patient Access functions across the King Lincoln and Short North Health Centers. This position provides frontline leadership, training, coaching, and daily operational support to Patient Access Representatives while maintaining direct patient-facing responsibilities.
The PAR Lead serves as the primary point of contact for registration, scheduling, patient access workflows, and operational issues that arise during clinic hours. ESSENTIAL JOB FUNCTIONS:

MAJOR AREAS OF RESPONSIBILITIES:
- Provide high quality case management for clients and their families by assisting them to access medical services, health insurance, Ryan White benefits, and other resources and services to improve health outcomes, housing stability, and employment and income attainment.
- Conduct comprehensive psychosocial assessments for people with HIV/AIDS seeking services at intake and complete update assessments each bi-annually and as needed. Medical Social Workers will assist clients in completing and submitting all necessary documentation related to these assessments.
- Develop, monitor, and evaluate individual case plans for each assigned client at intake, bi-annually, and as needed thereafter. Case Plans will address services provided to the client within Equitas Health, as well as services managed within the community by other providers.
- Function as a central and primary access point for financial assistance programs, including but not limited to Ryan White Treatment Modernization Act (Part B and C), HOPWA short term rental assistance, and other assistance programs as appropriate. Medical Social Workers will complete and submit paperwork as is needed to support client in maintaining these assistance programs.
- Assess client’s mental health needs and provide crisis intervention as necessary. Medical Social Worker's are responsible for completing lethality assessment documentation and consulting with Supervisors whenever a crisis occurs. Medical Social Workers will also reach out to community mental health services and consult with ongoing Mental Health and Therapy Providers as appropriate.
- Assist client with linkage to resources such as housing, respite, nutritional assistance, palliative care, chore assistance, transportation and social functions that help increase the client’s ability to remain independent in the community.
- Navigate community workforce programs and provide supportive services to clients that address the unique barriers to employment PLWHA may face in returning to work, understanding benefits eligibility, confidentiality and health management in the workplace.
- Identify and engage health care professionals in the region to provide quality services to HIV+ individuals and establish new relationships in collaboration with ODH. Medical Social Worker will refer Providers who seek a relationship with ODH to the appropriate contacts within ODH.
- Represent Equitas Health within the community, engaging other service providers and providing education about special needs associated with a client living with HIV/AIDS in the primary care continuum, mental health continuum, and other community resources.
- Works collaboratively within a multidisciplinary team.
- Maintain confidentiality of clients by adhering to Equitas Health Confidentiality Policy and Procedure, HIPAA, and other established professional standards and guidelines.
- Medical Social Workers are responsible to maintain documentation through Equitas Health, ODH, and other software systems. All documentation will be recorded and complete within two business days of provided service.
- Effective written and verbal communication skills. Ensure that action items and updates are provided to Supervisor proactively. Capture feedback from clients, staff, and community partners and communicate the information to the appropriate persons.
- Returns client, provider, and other stakeholder correspondence within 2 business days.
- Achieve productivity standards maintained by Equitas Health, including spending no less than 60% per month of hours worked directly engaging with clients, their families, and other informal supports.
- Participate in and complete Peer Review Audits monthly. Medical Social Workers will maintain scores of no less than 90% on monthly peer reviews.
- Coordinate with clients in order to maintain Active status through Ryan White and other programs. Medical Social Workers are responsible to have no less than 90% of their clients within date or identified as active in any given month.
- Responsible for accurate and timely completion of documentation in order to provide accurate data and reports to Equitas Health and its Board, as well as federal, state, and local governments.
- Attend trainings, as assigned, to improve case management skills related to written and verbal skills, putting theory into practice, and accurate documentation across multiple systems.
- Medical Social Workers will participate in Motivational Interviewing trainings and Learning Groups. As appropriate, Supervisors will schedule shadowing and review recorded visits between Medical Social Workers and clients in order to evaluate Motivational Interviewing skills.

POSITION SUMMARY
Reporting to the Clinic Manager, the Patient Access Representative (PAR) Lead serves as the operational lead for Patient Access functions across the King Lincoln and Short North Health Centers. This position provides frontline leadership, training, coaching, and daily operational support to Patient Access Representatives while maintaining direct patient-facing responsibilities.
The PAR Lead serves as the primary point of contact for registration, scheduling, patient access workflows, and operational issues that arise during clinic hours. POSITION SUMMARY:
The Care Right There (CRT) Therapist is responsible for providing comprehensive, client-centered mental health treatment to clients who are living with HIV/AIDS. Care Right There is a Ryan White Part A funded mental health treatment program focusing on meeting the needs of clients who have identified mental health needs but are unable to access clinic- or office-based treatment due to symptoms and other psycho-social stressors. The CRT Therapist provides treatment to clients in the home, in the community, and via telehealth in an effort to reduce symptoms and barriers that impact the client’s ability to participate in office-based outpatient therapy. - Collaborate with a multi-disciplinary team of professionals involved in caring for shared clients via Epic, email, phone, or face to face interactions.
- Complete comprehensive, client-focused diagnostic assessments, treatment plans and treatment evaluations.
- Assess appropriate level of care for clients and offer referrals when clinically indicated.
- Determine clients’ risk of harm, promptly complete safety plans when clinically indicated, and consult with supervisor when needed.
- Conduct regular follow-up with inactive clients to encourage participation in treatment.
- Provide community-/home-based treatment to clients who are unable to participate in traditional office-based services.
- Assist in development and facilitation of support groups and therapy groups.
- Demonstrate unconditional positive regard and conduct all aspects of job responsibilities with a focus on exceptional customer service.
- Maintain client records as directed by agency policies and procedures.
- Complete identified training to stay informed about current research and psychotherapy interventions.
- Assist with program development and the continued education of staff, students and community on relevant topics.
- Other duties as assigned by supervisor.
EDUCATION/LICENSURE:
- Masters degree in social work or counseling
- LSW or LPC licensure (LISW or LPCC preferred)
Knowledge, Skills, Abilities and other Qualifications:
- Post-Master’s experience providing mental health and/or substance use treatment
- Must have knowledge of, sensitivity to, interest in, and competence in cultural differences, HIV/AIDS, minority health, sexual practices; a demonstrated competence in working with Black, Indigenous, and other People of Color (BIPOC), LGBTQ individuals, and people living in poverty
- Strong written and oral communication skills
- Proficiency in all Microsoft Office applications and other computer applications
- Familiarity with EPIC or other Electronic Health Systems (EHR) preferred
- Reliable transportation, driver’s license, and proof of auto insurance
- Knowledge and adherence to professional standards and ethics
