JOB SUMMARY:
The Nurse Practitioner provides clinical care to patients, based upon his/her specialty training, through an interdisciplinary and collaborative approach that takes into consideration the psychosocial aspects of both health and disease.
MAJOR DUTIES AND RESPONSIBILITIES:

EDUCATION AND/OR EXPERIENCE:

WORKING CONDITIONS/PHYSICAL DEMANDS:
• Work is typically performed in a clinical and office environment. Travel among Chase Brexton Health Services clinics is required. Exposure to communicable diseases or hazardous material is possible, with little likelihood of harm if established precautions are followed.
• Ability to perform functions normally completed during the course of clinical care (i.e., sitting, standing, fine motor coordination), use of the keyboard for typing or editing of EMR notes, clinically evaluating and communicating plans to patients.
• Must meet FTCA requirements.

Behavioral Health encompasses prevention, intervention, and recovery from mental health and substance use conditions within the primary care setting across Chase Brexton Health Care.
MAJOR DUTIES AND RESPONSIBILITIES:

· Conducts brief, evidence-based and developmentally appropriate screenings.
· Conducts time limited assessments.
· Applies knowledge of evidence-based practice, including empirical bases of assessment, intervention, and other clinical applications, expertise and patient preferences to individuals, couples, groups and families to include adults, children and adolescents as assigned.
· Provides emergency assessment of patients in crisis and facilitates psychiatric hospitalization if needed.
· Assesses discharge readiness throughout the course of treatment.
Problem Solving and Decision Making
· Create and implement integrated treatment plans, ensuring access to organizational and external services within allotted time frames.
· Independently evaluates treatment progress and modifies planning as indicated, even in the absence of established outcome measures
Communication
· Provides consultation to medical providers, other CBHC staff, and other service providers to facilitate a coordinated continuum of care within the primary care setting.
· Participates in staff meetings as required.
· Verbal, nonverbal, and written communications are informative and well-integrated.

· Ensures progress notes are accessible to the patient's care team and considers that they may be accessible to the patient.

The Certified Nurse-Midwife assumes responsibility for the effective implementation of the philosophy, goals, policies and procedures of the health center and midwifery services. The nurse-midwife is responsible for adhering to performance goals, patient safety goals, and patient satisfaction goals set by the organization. o No intrapartum management/deliveries.
