POSITION SUMMARY
The Adult Nurse Practitioner/Physician Assistant is primarily responsible for providing comprehensive primary health care services through the identification, management and/or referral of the health problems and the maintenance of the client’s health by means of preventive care and health promotion. Will perform assignments which can vary from routine to exceptional in nature and are carried out with minimal guidance in accordance with clinic office practices and procedures. Adelante Healthcare is looking for Family and Internal Medicine Physicians to join our team who are passionate about providing care to the underserved communities in the metro Phoenix area. We were founded in 1979 when there was a need for healthcare for farm workers and migrants. Since that time we have grown and now have 9 clinics throughout Maricopa County (Phoenix and surrounding areas), and are continuing to grow as the demands for quality healthcare continues. We provide Family Medicine, Internal Medicine, Pediatrics, Dental and Behavioral Health.
Our Federally Qualified Health Centers provide care to a diverse patient population in a Patient Centered Medical Home model through multi-disciplinary care teams.
