The SHC staff is committed to recognizing the diverse needs of students, encouraging them to become equal partners in their healthcare and respecting the importance of family and community in the social and emotional lives and development of students.Summary: The clinic physician works interdependently with students, school staff, parents, other healthcare providers and healthcare team members to maintain a CCHC/SHC office that respects the dignity and worth of the individual and conveys an attitude of caring for students and their families physical, mental, oral and spiritual well-being. The clinic physician works to support the Patient Centered Medical Home (PCMH) concept which is a system of care in which a team of health professionals work together to provide all of the patient’s healthcare needs. CCHC utilizes technology such as electronic health records to communication and coordinate patient care and provide the best possible outcomes for our patients.The clinic physician will work with patients in reaching personal health improvement goals. They must be able to quickly assimilate to the patient’s culture, environment and goals. Relationships based on trust, an understanding of the patient’s health benefits program and the local health care community is required. Strong collaborative skills are necessary in order to make the best use of all resources available to the patient.The clinic physician will engage patients in personal health improvement, provide care coordination services and act as a resource and advocate for patient health care needs. The physician must understand the health impact that can be made by managing chronic conditions and providing patients unencumbered access to care.The clinic physician will foster an environment of collaboration and teamwork that promotes an outstanding patient experience.The clinic physician will manage, deliver and develop care coordination services for the health center’s patients.Essential Duties and Responsibilities include the following. Other duties may be assigned.· Manages care of patient in collaboration with other health professionals and members of the healthcare team in order to make decisions about treatment and attain health goals.· Examines patient, performs comprehensive physical examination and complies patient medical data, including health history and results of physical examination.· Documents patient information in the electronic health record.· Administers or orders diagnostic tests, such as x-ray, electrocardiogram and laboratory tests and interprets test results for deviations from normal.· Assesses social, cultural and family history of patients.· Performs short-term patient counseling and/or refers to appropriate counseling service.· Utilizes written medical protocols and/or in consultation with preceptor physician, plans medical and nursing therapies.· Differentiates between the normal and abnormal. Initiates treatment and/or refers patients to specialists if required.· Conducts screening for communicable diseases. Provides follow-assessment, documentation, evaluation and reporting if required.· Performs therapeutic procedures such as injections, immunizations, suturing and wound care and managing infections.· Develops and implements patient care plans, records progress notes and assists in provision of continuity of care.· Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living and health maintenance.· Coordinates care with hospitals, emergency rooms, consulting physicians and community resources.· Performs follow-up and coordination with extended care teams (specialists, ancillary care, etc.).· Provides an after visit clinical summary and review care plan with patients after each patient encounter.· Involves patient and family in planning patient’s care.· Educates patients about self-management tasks.· Assists in the overall management of chronic care patients by performing the following in conjunction with the other members of the healthcare team:Provides follow-up contact with patients as indicated to ensure compliance with prescribed care plan.Utilizes other care team members to telephonically advise per established protocols.Utilizes other care team members to perform pre-visit planning to ensure that necessary documentation and pre-visit tasks are completed prior to visit whenever possible.Assists patient with meeting short and long term goals for the patient and family/significant others as directed by a care plan.Assesses barriers to not meeting treatment goals and advises care team of impacts to care plan.Utilizes other care team members to provide direct chronic disease education to patients.· Documents patient problems and care coordination needs of the patient and family/significant others.· Identifies the need to involve other disciplines in the patient’s care.· Utilizes the use of electronic health records in capturing and utilizing clinical data.· Works collaboratively with senior management to set goals and objectives for the clinic including expansion of services.· Meets regularly with clinic staff to ensure care coordination, referral management and clinical workflow process are operating efficiently.· Attends and participates in morning team huddle to facilitate patient care.· Collaborates with other professional personnel and ancillary members of the healthcare team in determining care, treatment and general management of patients.· Provides community education as requested.· Participates in continuing education and in-service programs.· Participates in patient care quality assurance programs.· Supports an on-going quality assurance program in order to maintain and improve the quality of patient care provided at CCHC/SHC.· Works with other care team members to track and prompt patients for services du including but not limited to HgA1c, pap smears, immunizations, blood pressure checks and weight checks.· Attends and participates in staff meetings.· Adheres to CCHD and OSHA policies related to infection control procedures.· Must be or become MAT waived to provide substance abuse services to patientsCass County Health Clinic offers a competitive and comprehensive benefits package designed to support the well-being of our team.
