Nurse Practitioner - Family Practice

Greater Family Health, formerly Greater Elgin Family Care Center, is recruiting for a full-time family medicine Advanced Practice Professional (FNP or PA) for its Health Centers in Elgin, IL (373 Summits St., Elgin, IL. 60120) and Streamwood, IL (135 E. Irving Park Rd., Streamwood, IL. 60107).This practitioner provides comprehensive primary health care services to a primarily low-income population, focusing on adult medicine with some pediatrics. Well care, sick care and chronic disease management are common. The position provides outpatient care full-time 4 days a week, with 2 evenings until 7:30 pm, and 1 Saturday a month from 8:00 am - 5:00 pm. The position has no hospital rounding responsibilities and is responsible for practice call no more often than 1 week of every 35 weeks.As Greater Family Health (formerly Greater Elgin Family Care Center) is Joint Commission Accredited and PCMH certified, the practitioner is supported by a Care Team which includes a bilingual Medical Assistant, Clinical Operations Manager, Care Manager, Health Benefits Specialist, Clinic Coordinator and several additional professionals.
