In this role, the Community Health Worker (CHW) must be able to adapt services offered based on individual client and natural support system needs. Duties may include but are not excluded to processing health screenings, responding to warm handoffs, documenting within an electronic health record system, assisting clients to complete paperwork, researching community resources, offering individualized resources based on identified need, transporting and/or attending specialty appointments with clients in an agency vehicle, placing and following up on community referrals, and quickly re-prioritizing tasks. Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories). Half-time service is also eligible at a reduced award. Eligibility extends to NPs, PAs, CNMs, CRNAs, and physicians with qualifying education debt.
