The Drug & Alcohol Counselor provides direct client services, including screenings, assessments, evaluating level of care, diagnose for substance use criteria, screen for mental health symptoms, refer, treatment plan and document appropriately. The Counselor will provide counsel and support to patients suffering from drug or alcohol dependency, educate families in the best ways to help in the recovery process, and work with those participating in outpatient programs.
Qualifications:
- High School Diploma or equivalent required
- MHACBO Certified Alcohol and Drug Counselor I, II, or III status or equivalent
Primary Responsibilities:
- Complete ASAM assessment to determine appropriate treatment plan
- Facilitates groups using education, discussion and group process. Focus on emotional / social / and family issues relevant to clients suffering with substance use disorders
- Complete client documentation in a timely manner. Documentation to be completed within client’s EMR
- Provide regular status reports to referral sources and community partners. Unscheduled phone calls and emails may need to be sent/responded to referral sources and community partners
- Some departments may require participating in the collection of UAs, utilizing agency protocol.
- Participates in clinical supervision, including group and individual observation, QA review of documentation, and personal goal tracking highlighted in yearly evaluation. Attends minimum of 2 hours individual or group supervision per month

The Open Access Therapist provides access to integrated outpatient mental health treatment services that are based on medical necessity and that support and promote mental health and recovery. Therapists endeavor to a) Promote recovery, resiliency, wellness, independence and safety for individuals receiving addictions and mental health services and supports; b) Specify standards for services and supports that are person-directed, client-guided, culturally competent, and trauma-informed and wellness informed; and c) Promote functional and rehabilitative outcomes for individuals throughout a continuum of care that is developmentally appropriate.
The Open Access Therapist's primary responsibilities are to engage and respond in crisis intervention and facilitate access for mental health treatment at Adapt Integrated Health Care. They accomplish these responsibilities through assessment and prescription of treatment services that support rehabilitation, safety, and recovery through measurable outcomes in collaboration with clientele, and their natural supports when appropriate. Some of the interventions provided include comprehensive assessment; collaboratively developed Individual Service and Support Plan; individual and group therapy and skills training and development; case management; referrals, consultation, coordination and collaboration with community partners and providers; Discharge Summary and continuity of care through transfer and referral.
Qualifications:
- Master’s Degree in Social Work or Psychology, plus experience in a mental health setting is preferred.
- Must be 21+ and possess a valid driver's license.
- Current MHACBO eligibility/registration as a QMHP or current Oregon Licensing Board Associate Registration.
Primary Responsibilities:
- Provides treatment services through assessments and evaluations, including case management, individual and group treatment which promotes recovery, resiliency, wellness, independence, and safety for individuals receiving addictions and mental health treatment services and supports through prescription of outpatient treatment services that are client-centered, trauma-informed, Evidenced-Based Best-Practices whenever possible; collaborates with clientele and in consultation with team; completes Safety and Recovery Plans for those individuals that are priority one, those at risk, or for those having a history that indicates that a Safety and Recovery Plan would be of benefit.
- Performs many functions and treatment interventions to adult clientele at Adapt including crisis intervention, eligibility, access, and enrollment services; provides comprehensive assessment and the collaborative development of Individual Service and Support Plans (ISSP); coordinates Individual and Group Therapy and Skills Development and Training; provides Case Management services and supports and consultation, referral, and coordination with agency partners and providers; coordinates discharge planning that includes supports and active planning for provision of continuity of care; coordinates and provides intervention in provision of crisis services as necessary in consultation with Crisis Team and supervisor and/or program manager as soon as this is possible and appropriate; completes SERFs and Incident Accident Reports when appropriate; follows established policy and protocol.
- Creates access that enables clientele to obtain needed resources, supports, and treatment services (e.g., substance use, medical) through referral, consultation, coordination, case management, and advocacy; provides consultation and training for and with clients, staff, professionals, other services agencies, and the public; attends unit meetings and workshops; develops, modifies, and/or obtains training programs and materials in consultation with the program manager.
- Maintains and completes required records, reports, and documentation as required by departmental policy and state law; provides documentation and written communications.
