Nurse Practitioner / Physician Assistant

Practice Info

• Works collaboratively with all members of the health center to build a supportive team
• On-site at a DAP Health clinic
• Operates in an office setting
• Requires frequent times of sitting, standing, repetitive motion, and talking

Responsibilities

• Assess and interpret medical and psycho-social histories; perform physical examinations; identify problems; formulate a plan of treatment; provide education and referral; and evaluate the effectiveness of the interventions prescribed
• Independently assess and treat patients as clinically indicated
• Perform or order laboratory tests and interpret the results as part of health management
• Order medications
• Follow established treatment guidelines as indicated by national guidelines or Health Center protocols
• Meet clinical quality performance measures (UDS and HEDIS measures)
• Conduct health teaching and counseling during the medical encounter, as appropriate
• Complete the client's clinical record documentation in a timely manner specific to the applicable policies and procedures of the health center

Compensation

• New Hire Bonus of $10,000, less payroll deductions and tax withholdings, upon signing a Repayment Agreement
• This New Hire Bonus will be paid on a DAP Health payroll date within the first thirty (30) days of your hire date of employment
• If employment is terminated within two (2) years after receiving the New Hire Bonus, a repayment amount will be due and will be prorated based on the number of calendar days in which you were employed with DAP Health following the date you received the New Hire Bonus
• Alternatively, you may choose to receive the New Hire Bonus after one (1) full year of continuous employment (the "Delayed Bonus"), after which the New Hire Bonus will be earned and paid and there will be no repayment obligation

Shift & Schedule

• Available for all shifts and, when required, able to work evenings and weekends

Requirements

• Visual acuity and manual dexterity to interface with the computer and perform medical procedures as indicated
• Ability to communicate effectively with all levels of individuals both internally and externally
• Maintain CMEs/CEUs required for licensure
• Master's degree from an accredited physician assistant or nurse practitioner program required
• Valid CA Physician Assistant or Nurse Practitioner certification required
• Valid CA DEA license required
• Minimum of one (1) year of experience as a Nurse Practitioner (for NP)
• Licensed to practice as a CA Nurse Practitioner and currently registered as a Registered Nurse in the State of California (for NP)
• Certified by ANCC or AANP (for NP)
• Valid BLS certification obtained through the American Heart Association or American Red Cross upon hire
• Current State of California Physician Assistant License (for PA)
• Current valid Federal Controlled Substance Registration Certificate (for PA)
• Minimum of one (1) year of experience as a Physician Assistant (for PA)
• Experience working for a non-profit organization or clinic preferred (for NP)
• Experience with Epic preferred (for NP)
• Experience in a Federally Qualified Health Center (FQHC) preferred (for NP)
• Experience with the LGBTQ and Transgender populations preferred (for NP)
• Experience with the homeless population preferred (for NP)
• Experience with HIV+ persons preferred (for NP)
• Bilingual in Spanish preferred (for NP)
• Requires current and valid driver's license and current personal auto insurance
• Ability to lift up to 40 pounds and move from place to place
